It sounds too simple to work — almost dismissive. But the moment that cold water hits your skin, something shifts. Your heart rate drops. Your breathing slows. What Is the Cold Water Trick?
The "cold water trick" refers to using cold temperature on the face or body to rapidly shift your nervous system out of a fight-or-flight state. It is not a new wellness trend — it is rooted in a reflex that every mammal on Earth shares.
When cold water contacts your face, it triggers what physiologists call the mammalian dive reflex — an ancient, hardwired response that slows your heart rate, redistributes blood flow, and activates your parasympathetic nervous system. The reflex exists because it protects the brain during underwater submersion, but you do not need to dive into a lake to trigger it. A bowl of cold water, a splash from the tap, or an ice pack held against your cheeks will do.
This technique is not just folk wisdom. It is a core component of the TIPP skills taught in dialectical behaviour therapy (DBT), one of the most widely practised evidence-based therapies for emotional regulation. The "T" in TIPP stands for Temperature — specifically, using cold to rapidly change your body's physiological state when emotions feel overwhelming. DBT clinicians recommend holding your face in cold water or pressing a cold pack to your forehead and cheeks for about 30 seconds.
How Cold Water Activates Your Vagus Nerve
The vagus nerve is the longest cranial nerve in your body. It runs from your brainstem down through your neck, chest, and abdomen, connecting your brain to your heart, lungs, and gut. It is the main highway of your parasympathetic nervous system — the branch responsible for calming you down after a threat has passed.
When cold water touches your face, it stimulates the trigeminal nerve, which innervates the forehead, cheeks, and nose. The trigeminal nerve sends a signal to the brainstem, which in turn activates the vagus nerve. How to Try Cold Exposure for Anxiety: Step by Step
You do not need an ice bath, a cold plunge tub, or any special equipment. Here are three practical ways to use cold exposure for anxiety, ordered from gentlest to most intense.
1. The Face Splash (Fastest Option)
- Turn on the cold tap — as cold as it comfortably goes.
- Cup cold water in your hands and splash it firmly across your forehead, cheeks, and around your nose. These are the areas where the trigeminal nerve is most concentrated.
- Repeat two or three times, pausing between splashes to notice your heart rate settling.
This takes under a minute and can be done in any bathroom. It is the version most often recommended in DBT.
2. The Cold Pack Method (Best When You Cannot Get to a Sink)
- Grab a cold pack, a bag of frozen vegetables, or a wet flannel that has been in the fridge.
- Hold it against your forehead and cheeks for 30 seconds. If you can, lean forward slightly and hold your breath for 15 seconds — breath-holding intensifies the dive reflex.
- Remove the pack and sit quietly for a minute, breathing slowly.
This version is useful at work, on a plane, or anywhere a tap is not available.
3. The Cold Shower Ending (For Building a Regular Practice)
- Take your normal warm shower.
- In the last 30 to 60 seconds, turn the water to cold. Let it run over your face, neck, and chest.
- Focus on slow, steady exhales as your body adjusts to the temperature change. The urge to gasp is normal — breathe through it.
- Step out and notice how you feel: alert, grounded, and often calmer than when you stepped in.
A 2016 randomised controlled trial by Buijze et al., published in PLOS ONE, studied over 3,000 participants who added a cold shower ending (30, 60, or 90 seconds) to their daily routine. Participants reported increased energy and a perceived improvement in quality of life, and the cold shower group had a 29 percent reduction in sickness absence from work compared to controls.
What the Research Actually Shows
It is important to be honest about where the science stands. The dive reflex and its effect on heart rate are well-established physiology — this is not disputed. What is less certain is whether cold exposure produces lasting benefits for anxiety beyond the immediate moment.
A 2024 randomised controlled trial tested the Wim Hof Method (cold showers combined with specific breathing techniques) against warm showers with rhythmic breathing in 84 women with moderate depressive symptoms. After eight weeks, both groups improved by roughly the same amount — depressive symptoms dropped by 24 percent, anxiety fell by 27 percent, and stress decreased by 20 percent. The cold shower group did not outperform the warm shower group on any primary measure.
This does not mean cold exposure is useless for anxiety. It suggests that the breathing component and the ritual of deliberate self-care may be as important as the cold itself. A 2025 systematic review protocol published in Frontiers in Psychiatry noted that while anecdotal evidence and preliminary studies suggest benefits, the scientific evidence remains fragmented and more rigorous research is needed.
What we can say with confidence:
- The dive reflex is real and produces measurable, immediate parasympathetic activation.
- Cold facial contact is a legitimate in-the-moment tool for reducing acute emotional distress — this is why it is included in DBT.
- Cold showers may improve mood and energy, but the evidence does not yet show they are more effective than other active relaxation practices.
- Cold exposure is not a treatment for anxiety disorders. It is a tool, not a therapy.
When Cold Exposure Might Not Be Right for You
Cold exposure is generally safe for healthy adults, but there are important exceptions:
- Cardiovascular conditions. Sudden cold immersion triggers a cold shock response — a sharp gasp, a spike in heart rate and blood pressure. For people with heart disease, uncontrolled hypertension, or arrhythmias, this can be dangerous. Check with your doctor first.
- Raynaud's disease. If you have Raynaud's, cold exposure can trigger painful vasospasm in your fingers and toes.
- Cold urticaria. Some people develop hives or swelling in response to cold temperatures.
- Pregnancy. The evidence on cold immersion during pregnancy is limited. Speak with your healthcare provider.
If cold exposure feels unpleasant, distressing, or makes your anxiety worse, stop. The goal is to activate a calming reflex, not to endure discomfort. A gentle face splash is enough — you do not need to prove anything.
